CHOICES
Review
what benefits are right for you and any dependents
you elect to enroll. Remember many choices (noted
with a *) are "pretax", meaning that
you do not pay state, federal income taxes on
those dollars or social security, further reducing
your cost.
New
for 2011-2011, New Haven is continuing to insure all benefit
eligible employees for $10,000 in Life
Insurance and $10,000 in AD&D through
Prinicipal Life Insurance. This benefit will also
provide an Employee Assistance Program funded
by New Haven's premium, an additional (confidential)
resource to you to assist with life challenges
(family issues, child/elder care needs, conflicts
at work, health or dependency issues - all at
no cost to you. Look for details below. (Additional
voluntary life, for you and dependents, is available
for purchase at affordable rates).

A.
UNITEDHEALTHCARE
The UnitedHealthcare (Pacificare)
Plans offer
choice (several groups are in this plan -
see attached for details) in doctors and/or networks
in addition to all the benefits of the HMO plan.
Choose your own doctor and then associate with
their group. You may change to any doctor within
their network (monthly basis).
- Low
Option (Pacificare)
- An affordable HMO with a $20 office copay,
the option provides a variety of network options
and doctor choices. Premiums are lower because
you share in any Hospitalization costs up to
the 30% copay, not to exceed the annual (individual)
copayment maximum of $3,000. Prescriptions
are covered at $15 for generic, $35 for brand,
and $50 for non-formulary. This plan does not
have Chiropractic benefit.
- High
Option (Pacificare) - A higher priced HMO
option also with a $20 office copay with your
Primary Care Provider. Hospitalization is $250 per admit.
Same prescription
coverage as above - $15
for generic, $35 for brand, and $50 for non-formulary.
Mail order prices save. This plan offers Chiropractic
AND Acupuncture. .

B.
KAISER -
Kaiser
offers a self contained environment within the
HMO facilities. While many emergency and clinics
are available in North County, the primary hospital
location is in Mission Valley. You may choose
any doctor within their network.
- Low
Option (Kaiser) - This
affordable HMO option changes slightly from
last year. It offers healthcare, within the
Kaiser network, with a $30 office copy
and a $500 per day hospitalization cost, not
to exceed the annual (individual) copayment
maximum of $3,000. . Generic
and Brand prescriptions are covered at a copay
of $15 and $30 respectively. Mail
order saves dollars.

Option (Kaiser) -
This option is a more expensive healthcare
HMO option with a $15 copay and a lower
$250 cost for hospital admission. Again, Generic
and Brand prescriptions are covered at a copay
of $15 and $30 respectively. Mail
order saves dollars. This plan offers Chiropractic.

DENTAL
PLANS*
New Haven
offers several choices in dental coverage, Vision
discounts are also available through Principal.
Compare and see which place best meets your dental
needs.
An HMO Plan
(1) With no deductible or lifetime maximums,
this plan affords you the least expensive networks
and allows you to simply pay a copayment through
a contracted dentist (through the
California Dental network). Orthodontic
benefits are only available on the HMO plan. Call

DENTAL to find a provider near you.
POS Plan (including PPO)
(2) If
you prefer a dentist of your choosing, we also
offer choices through the Principal
network. Both plans include a cosmetic & implant
benefit AND allow for 4 cleanings per year (12
month period). The calendar year maximum has also
been increased to $1,500
Pick
either a Low of High POS Plan - Option 1 or Option
2, with fee discounts to dentists who are
EPO providers. It is like having 3 options in
one - with the greatest benefits to you in the
the EPO networks. Prefer, a dentist you have had
forever? - no problem. This plan also reinburses
for non-network dentists (but % of reimbursement
varies based on affiliation). Learn more about

EAP
PROGRAM - New for this year, these core
services are provided by Magellan
Health Services to offer e employees and their
dependents, important resources for everyday challenges.
Services include telephone and individual consultations
regarding parenting issues, conflicts at home/work,
child/elder care needs, emotional/stress related
issues, and even drug/alcohol dependencies.
VISION
PLAN*
This plan,
offered by UnitedHealthcare, adds extended benefits
for vision coverage as well as frame and lens
discounts. The network can be accessed through the website or by calling 1-800-839-3242. No copayment is provided by NH for
this benefit; however, this program offers you a national network in oth private practice and retail chains for free annual exams and lenses, annually, with frame discounts every 24 months. (Note: all of the above medical
plans offer vision screening and discounts to
selected providers for those wearing glasses.)
LIFE
INSURANCE
Offered by Unun
you have a choice of both term life coverage and/or
Accidential Death and Dismemberment at affordable
rates. Check out the rates
here for additional term life - it is shown
as semi-monthly numbers (so you need to double
to show monthly totals on our form), and is charged
by whether you are a smoker or non-smoker.
Term
life provides guaranteed benefits (without medical
information) of up to $100,000 for yourself (and
up to $30,000 your spouse, if desired) in the event of the death.
The premium
is very affordable but still age rated (increasing
as one gets older) and requires completion of
the medical information only if applied for in
excess of NH's guarantee issue of $100,000. Coverage
may be as much as $500,000. You may also purchase
additional AD&D (as long as that does not exceed 5X income). Different
limits may apply to your spouse and/or children,
but check this out if you wish additional insurance
coverage. Remember, New Haven already insures
you for $10,000 so this is in addition to that
(yet is portable* - where the Group Life is not).
AD&D provides additional benefits if any covered
parties die as a result of an accident. AD&D
is priced, additionally, at a rate / $1,000 for
employees of .000285 (that's right - $100,000
costs you $2.85/month).
* Plans that are portable
may usually be continued individually by you at
no upcharge if you disengage from employment from
New Haven.
403B
or Roth IRA*
Use
pretax dollars to save for tomorrow aftertax!
This is a
retirement savings plan and the 403B option
shelters today's income from tax while a Roth
IRA is with aftertax dollars. With either plan,
you choose from a variety of investment options
through a broker, retaining the flexibility of
your voluntary deductions, up to the annual maximums
allowed by IRS (2010-2011 annual deductions may
be up to $16,500 with catch-up contributions of
$5.5K allowed for those over 50). To indicate
your interest in a 403B account, simply email
our broker your contact information and they
will schedule an appointment with you to investigate
and/or set up this account. We will begin deductions
once the account is set up and you have authorized
us to debit payroll.
FLEX
PLANS*
You
may also defer dollars into either an Unreimbursed
Medical Account (URM - presently capped at
a maximum of $3,000/plan year) and/or Dependent
Care Account to save taxes on qualifying expenses
that you anticipate spending during the eligible
plan year. Complete all sections of the Flex Plan
Enrollment Form.
A
Flex
Plan allows you to save taxes on any funds
that you defer into this plan, a great tax benefit!
Estimate carefully and be sure that anticipated
expenses are eligible as receipts are required
to access your funds and must be dated for eligible
expenses incurred within the qualifying time frames
or you forfeit any unused dollars. Refunds
are processed through AFLAC (to maintain confidentially
of your medical history) and then credited back
to you through your payroll account. Still a great
way to maximum your earnings!
VOLUNTARY
SHORT TERM DISABILITY - You may apply for additional Short
Term Disability benefits (in addition to those
provided under the state plan) if you wish to
assure income protection up to your annual salary
in the event of a disability by either accident
or health conditions. Varying amounts of coverage,
rates,
and waiting periods apply, depending on your
age and requested coverages.
VOLUNTARY
ACCIDENT INSURANCE - Should you be injured
in a covered accident on or off the job, this
insurance provides you wish benefits for injuries,
treatment, hospitalization, even transportation
and lodging. See HR for details.
