Non-Exempt Employee (paid hourly)
Eligible after 3 full months as of 1st of month.

Non-Exempt Employee
All completed paperwork should be completed by the
middle of 3rd month with us!
Example: If your hire date is 2/15, you benefits start 6/01
Paperwork completion date = 5/15
Effective Date - 6/01
Paperwork received late (on or after the effective date of coverage) is not guaranteed to be eligible for processing.


Exempt Employee (paid a salary)
Eligible 1st of month following 30 days from Hire Date.

ubmit your paperwork between your first day of work and 15 days prior to the effective date, whichever is first.

Example: If your hire date is 2/01 - submit paperwork by 2/15 to be eligible 3/01.
Enrollment Changes Due at within 30 days of event (loss of coverage, dependent changes), to be effective the first of month following change date.
