Requesting Time OFF?
Please use the Time Off Request Form
to notify and seek approval for planned absences.
Absence requests will be reviewed based
on many factors in addition to your request/need, i.e.
agency need, attendance history, house stability.
Extended/unusual absences will be reviewed
for Leave of Absence Eligibility.
(Note: if you wish to receive Vacation Time,
if available,
be sure to attach a time sheet
with the time off request).
