We know benefits are important and our proud of the options we can offer you! Because we place a high priority on your healthcare benefits, New Haven pays $250/month ($3,000/year) towards the cost of your coverage if you are a full-time regular employee! A great addition to your compensation package.

Below is some information regarding the following benefits:

Review what benefits are right for you and any dependents you elect to enroll. You may only elect coverage when you are first eligible or at open enrollment (unless a you qualify for a based on a defined "change in status"). Remember many choices (noted with a *) are "pretax", meaning that you do not pay state and federal income taxes on those dollars, further reducing the cost of your benefit.

  • Non-Exempt Employees - Submit paperwork the beginning of the month prior to your 90th day of employment.
    (You are eligible the first of the month following 90 days of employment)
  • Exempt Employees - Submit paperwork at time of hire.
    (You are eligible on the first of the month on or following your hire date).
  • Re-Hires / Changes of Status - Contact HR. You may be eligible immediately but must notify HR to verify eligiblility and complete paperwork within 30 days of any changes.

Once you have reviewed all of the options offered, be sure that you submit information (an election form AND all enrollment applications) to HR in time to meet your enrollment deadlines and allow adequate time for processing.

MEDICAL PLANS*
Compare here. Pick from 1 of 4 plans offered through either Pacificare or Kaiser. Pacificare Plans offers choice (major groups are in this plan - see their web site for details) in doctors and/or networks in addition to all the benefits of either an HMO or a PPO plan. Kaiser offers a self contained environment within an HMO facility. Both offer comparible plans and excellant doctors/facilities.All plans offer Chiropractic benefits at affordable copays and Behavioral Health Benefits. Brief descriptions of options offered are highlighted below. More details can be found on the plan descriptions highlighted.

  • Option 1 (PacifiCare) - An affordable HMO with a $25 office copay. Hospitalization requires a 30% copay up to a annual (individual) copayment maximum of $2,000. Prescriptions are covered at $15 for generic, $30 for brand, and $45 for non-formulary. This plan does not have Chiropractic benefit but this plan is the most affordable at $16.11 per payroll.
  • Option 2 (Pacificare) - A medium priced HMO option with a $15 office copay. Hospitalization is $250 per admit. Same prescription coverage as above. Your premium cost per pay period is $36.26 (less taxes).
  • Option 3 - This PPO Option was withdrawn.
  • Option 4 (Kaiser) - Similar to the Pacificare Plan- Option 2, this HMO option is identical to last year's plan and offers care, within the Kaiser network, with a $25 office copy and a $250 hospitalization cost per admit. Generic and Brand prescriptions are covered at a copay of $10 and $25 respectively. Your premium cost per pay period is $18.95 (less taxes), nearly the same as the lowest price plan in PacifiCare.
  • Option 5 (Kaiser) - This option is a HI end HMO with a $10 copay and no cost for hospital admission. Again, Generic and Brand prescriptions are covered at a copay of $10 and $25 respectively. Your premium cost per pay period is $29.90 (less taxes).

DENTAL PLANS* - Same as last year, Golden West provides us with 3 plan choices - two PPO plans (HI & Low) and a PrePaid Plan (similar to an HMO). Compare and see which place best meets your dental needs.

VISION PLAN* - This plan, offered by Pacificare, adds extended benefits for vision coverage as well as frame and lens discounts. (Note: all of the above medical plans offer vision screening and discounts to selected providers for those wearing glasses.)

LIFE INSURANCE - Offered by Unum, you have a choice of both term life coverage and/or Accidential Death and Dismemberment at affordable rates.

Term life provides benefits in the event of the death of a covered employee, spouse, or child. The premium is age rated (increasing in 5 year increments as one gets older) and requires completiion of the Evidence of Insurability form if applied for in excess of NH's guarantee issue of $60,000 or if applied for any time other than when one is first eligible for benefits. AD&D provides additional benefits if any covered parties either die as a result of an accident. Plans are portable and may be continued individually by you whether or not you are associated to the group.

403B* - Use pretax dollars to save for tomorrow. This is a retirement savings plan and shelters today's income from tax. You choose from a variety of investment options through a broker, retaining the flexibility of your voluntary deductions, up to the annual maximums allowed by IRS (2007 annual deductions may be up to $15,500 with catch-up contributions allows for those over 50). To indicate your interest in a 403B account, simply email our broker your contact information and they will schedule an appointment with you to investigate and/or set up this account. We will begin deductions once the account is set up and you have authorized us to debit payroll.

FLEX PLANS* - You may also defer dollars into either an Unreimbursed Medical Account (URM - presently capped at a maximum of $3,000/plan year) and/or Dependent Care Account to save taxes on qualifying expenses that you anticipate spending during the eligible plan year. Complete all sections of the Flex Plan Enrollment Form.

A Flex Plan allows you to save taxes on any funds that you defer into this plan, a great tax benefit! Estimate carefully and be sure that anticipated expenses are eligible as receipts are required to access your funds and must be dated for eligible expenses incurred within the qualifying time frames or you forfeit any unused dollars. Refunds are processed through AFLAC (to maintain confidentially of your medical history) and then credited back to you through your payroll account. Still a great way to maximum your earnings!

VOLUNTARY STD - You may apply for additional Short Term Disability benefits (in addition to those provided under the state plan) if you wish to assure income protection up to your annual salary in the event of a disability by either accident or health conditions. Varying amounts of coverage, rates, and waiting periods apply, depending on your age and requested coverages.

VOLUNTARY ACCIDENT INSURANCE - Should you be injured in a covered accident on or off the job, this insurance provides you wish benefits for injuries, treatment, hospitalization, even transportation and lodging. See HR for details.
